Swain County Health Department Programs (Spanish Translation)

 

ADULT HEALTH PROGRAMS

Adult Health Clinic

Adult Health Clinic is for men and women 18 years and older for a complete physical. This service is by appointment only.

 

Communicable Disease

TB, AIDS, and other Communicable Diseases, provides counseling and assistance in accessing medical and support services.

Sexually Transmitted Disease STD’s Clinic provides testing for a variety of sexually transmitted disease. Educational counseling and preventive measures are included. Treatment for certain STD’s is provided at no charge.

 

Eye Services

A financial screening for eyeglasses is available with an appointment every Wednesday at 1:00 p.m. This service is sponsored by NC Commission of the Blind.

 

General Clinic

General clinic provides blood pressure, blood sugars, weight, and immunization checks per patient’s request. Other lab work is provided per doctor’s request. No appointments necessary for these services.

 

WOMEN'S HEALTH PROGRAMS
Women’s Health programs provides health care screening services and routine examinations.

 

Breast & Cervical Cancer Control Program (BCCCP)

Clinic is held each month. Women age 50 and older. Includes a physical examination and referral for a mammogram. Eligibility based on income.

 

Family Planning

 This service provides physical examinations, counseling, birth control methods and lab tests. Fee based on family size and income if not Medicaid eligible. Insurance is accepted. Family Planning is an appointment only service.

Pregnancy Testing
Provides counseling, testing, and referrals.

Pregnancy testing is free.

 

Pregnancy Care Management (PCM)

Pregnancy Care Management (PCM) is a program coordinated by a registered nurse for prenatal patients who need help with access to medical, social, educational or other services in the community.

 

ENVIRONMENTAL HEALTH
Promotes and protects public health through routine inspection of restaurants, motels, schools, swimming pools, day cares, etc. Proper design, inspection of sewage disposal systems, water quality sampling, rabies investigation, meth lab cleanup oversight and Environmental Health complaint investigations.

HEALTH EDUCATION
Health Education services are provided to promote healthy lifestyles. Health educators and other staff provide counseling for individual clients and programs for schools, community groups and businesses.

Partnership for Health-Healthy Carolinians is a grass roots organization whose mission is to work with citizens to identify the health needs of our community and develop local solutions to promote health. The organization works primarily through various health focused task forces made up of community volunteers. The current task forces include Healthy Lifestyles, Teen Pregnancy Prevention and Tobacco Prevention. These groups meet monthly and address issues in various ways, from physical changes in the community like a new walking trail to policy changes like advocating for a 100% Tobacco Free School System. All task forces are open to the public and invite concerned citizens to get involved by joining and participating in creating a healthier community. The Partnership is a certified Healthy Carolinians organization and works with the State of North Carolina to address the states 2010 initiatives. For more information please contact:

WOMEN, INFANT and CHILDREN

Women’s, Infants and Children (WIC) is a supplemental food program for women, infants and children. You may be eligible if you are pregnant, breastfeeding, or have a child less than 5 years of age. ***Discrimination Statement - This agency is an equal opportunity provider.***WIC is available to pregnant, breast feeding and postpartum women, infants and children up to age five. To participate, these persons must:
-Live in North Carolina.
-Have a family income less than 185% of the U.S. Poverty Income Guidelines. A person receiving Medicaid, Work First Families Assistance (TANF), or assistance from the NC Food and Nutrition Services automatically meets the income eligibility requirement.
-Be at nutritional risk. A nutritionist or other health professional makes the nutritional risk assessment at no cost to the participant, usually at the local WIC office.
-WIC Income Guidelines -Once the size and gross income of the economic unit (or household) have been determined and documented, staff use the poverty income guidelines included in the chart below to determine income eligibility. The guidelines are published by the U.S. Department of Health and Human Services (DHHS).

Breastfeeding Support Project assembles classes, books, video tapes, breast pumps, one-on-one instruction and support are provided for women who would like to, or are currently, breastfeeding.

 

CHILD HEALTH
Child Health is a referral service for uninsured children birth through 21 years of age to Sylva Pediatrics for routine checkups only. Authorizations for appointment are made by Swain County Health Department Staff.

Care Coordination for Children (CC4C) is a tracking program for infants and children showing evidence of risk for developmental, social, or medical problems.

Immunization Tracking and Services provides services for the purpose of tracking children birth through 2 years of age. It will insure compliance of the required immunization schedule and required childhood immunizations at designated intervals of life. Also provides adolescent and adult immunizations upon request or as recommended.

Car Seat Safety: The Department has one Certified Child Passenger Safety Technician who teaches parents about proper installation for compliance with the N.C. Child Restraint and Seat Belt laws. Appointments are scheduled on an individual basis.

Smart Start program provides nursing services, immunizations, dental, MTI vision screenings, etc., and education programs for children, teachers and parents in daycares. Welcome Baby packages distributed during home visits to Swain County expectant mothers. Fluoride Varnish applications to children 1-5 years old.

Children’s Special Health Services (Referral Services)- this program provides comprehensive treatment services for persons under the age of 21 with specific conditions. Patients must be Medicaid eligible. However, this program covers some services that Medicaid does not.

 

COMMUNITY ALTERNATIVES PROGRAM FOR DISABLED ADULTS (CAP/DA)
Program provides services to elderly and disabled adult residents facing Nursing Facility Care, enabling them to remain safely and independently at home as long as possible. Includes basic household tasks, meal preparation, personal care, must be Medicaid eligible. For more information call (828) 488-3792x2019.

HOME AND COMMUNTIY CARE BLOCK GRANT

Home and Community Care Block Grant program provides services to individuals 60 years of age or older. The services include basic household tasks, meal preparation and personal care services. Individual does not have to be Medicaid eligible. For more information call 828-483-3792x2020.

 

IN-HOME AIDE SERVICES

Service providing direct patient care and emotional support. Aides provide assistance with day-to-day tasks such as bathing, dressing, feeding and home management. Aides are trained to meet various levels of required care. For more information call 828-483-3792x2020.


IN-HOME CARE FOR ADULTS (IHCA)
A service which provides assistance and support for persons with disabilities, living independently in the community. This includes the elderly, pregnant women and others with special health care needs. Must be Medicaid eligible. For more information call 828-488-3792x2019.


HEALTH WATCH SERVICE

A telephone alert system for emergency response. Medicaid or Private Pay.

 

CAP/CHOICE

Program which allows elderly and disabled adults the opportunity to direct and have increased control over services and supports to meet their needs. Must be Medicaid eligible and/or currently receiving CAP/DA services. For more information, call 828.488.3198 x 2019.
